We contacted the vendor and they responded, "We want to assure you that all our parts undergo rigorous inspection and careful packaging before shipment. Responded back to the vendor that we tore the box apart, no bolt. They have gone silent. Update, vendor has offered a partially refund, guess I'll head to Home Depot and look for a replacement bolt.
